Dear Colleagues

 

This is to let you know that two slight amendments have been made to the ‘Guidance to creating and maintaining a Professional Learning Portfolio’ document on the FPH website.  They are as follows:

 

  1. On page 16 in the first column in the last two rows on the work place assessment sheet In the Learning portfolio.  The term ‘Educational Supervisor’ has been changed to ‘Project supervisor’.

 

  1. On page 17 above the last box where the Educational Supervisor needs to sign and date. The specific dates that a supervisor's report covers has been amended. 

 

  1. On pages 34 and 36, A ‘reflective notes’ column has been inserted into the forms for Study Leave and Presentations and the forms have been changed into a landscape position.
